Job description
VitalCaring Group is seeking a Home Health Physical Therapist to help patients regain mobility and independence in the comfort of their homes. You’ll work autonomously within a compassionate team, coordinating with nurses and physicians to ensure seamless care.
You will evaluate functional status, develop personalized plans, educate patients and families, and document progress in the EMR. PT license and CPR are required, with 1–2 years of home health experience preferred.
