Job description
Optimal Care is seeking a Home Health Registered Nurse to deliver skilled nursing care in patients' homes, supporting recovery and chronic disease management while promoting independence. You will manage a caseload from admission through discharge, conduct assessments, develop care plans, and coordinate with physicians and an inter-disciplinary team.
