Job description
Pine Park Health is seeking an experienced, independently licensed NP (California NP-103) to join our team in the East Bay San Francisco market (Oakland, Walnut Creek, Concord). You’ll autonomously manage a panel of older adult patients within a value-based primary care model, delivering onsite care in Senior Living Communities and leading a multidisciplinary team to improve outcomes.
