nWhat you'll do...
nIndependent Optometrist – Sam’s Club
nLaunch your services in Sam’s Clubs!
nAs an Independent Optometrist leasing space inside our Sam’s Club, you will take a leading role in patient care with respect to eye and vision care as well as general health and well-being.
nAs an independent lease holder, you have the autonomy to:
n• Establish your days of practice
n• Establish your hours of operation
n• Control your patient schedule
n• Set your fee schedule
n• Keep 100% of exam/professional fees
nFor a fair market value, you can opt into:
no Equipment/instrumentation
no Administrative and/or Opt-Tech support services (if state allows)
nMinimum Qualifications for a lease agreement:
n• Doctor of Optometry (OD) from an accredited optometry school
no Valid state optometry license
n• Ophthalmologist (MD or DO) from an accredited school
no Valid state ophthalmology license
n• General Liability Certificate
n• Professional Liability Insurance Certificate
n• W-9
