Position Summary...
nWhat you'll do...
nLaunch your services in Walmart stores!
nAs an Independent Optometrist leasing space inside our Walmart stores, you will take a leading role in patient care with respect to eye and vision care as well as general health and well-being.
nAs an independent lease holder, you have the autonomy to:
nnnEstablish your days of practice
n
nnEstablish your hours of operation
n
nnControl your patient schedule
n
nnSet your fee schedule
n
nnKeep 100% of exam/professional fees
n
n nFor a fair market value, you can opt into:
nnnEquipment/instrumentation
n
nnAdministrative and/or Opt-Tech support services (if state allows)
n
n nMinimum Qualifications for a lease agreement:
nnnDoctor of Optometry (OD) from an accredited optometry school
n
nnValid state optometry license
n
nnOphthalmologist (MD or DO) from an accredited school
n
nnValid state ophthalmology license
n
nnGeneral Liability Certificate
n
nnProfessional Liability Insurance Certificate
n
nnW-9
n
n nME & VA license agreements are with Gumberg Asset Management Company
