nLaunch your services in Walmart stores!
nAs an Independent Optometrist leasing space inside our Walmart stores, you will take a leading role in patient care with respect to eye and vision care as well as general health and well-being.
nAs an independent lease holder, you have the autonomy to:
nEstablish your days of practice
nEstablish your hours of operation
nControl your patient schedule
nSet your fee schedule
nKeep 100% of exam/professional fees
nFor a fair market value, you can opt into:
nEquipment/instrumentation
nAdministrative and/or Opt-Tech support services (if state allows)
nMinimum Qualifications for a lease agreement:
nDoctor of Optometry (OD) from an accredited optometry school
nValid state optometry license
nOphthalmologist (MD or DO) from an accredited school
nValid state ophthalmology license
nGeneral Liability Certificate
nProfessional Liability Insurance Certificate
nW-9
