Job description
Bloom Healthcare is seeking a Primary Care NP/PA to deliver in-home and community-based primary care to medically complex seniors in Fort Collins, CO. You will provide relationship-based care with autonomy and support from Bloom’s interdisciplinary team and advanced data systems.
You will diagnose and manage chronic and acute conditions, address social determinants of health, and coordinate timely post-discharge visits with a focus on patient-centered outcomes.
