Job description
Bloom Healthcare is seeking a Primary Care NP/PA to deliver relationship-based primary care to homebound seniors in Parker and surrounding communities. You’ll practice with independence yet rely on a robust interdisciplinary team and advanced data systems to guide decisions.
You’ll manage chronic and acute conditions, address social determinants of health, and coordinate care to improve outcomes while maintaining a patient-centered, unrushed approach.
