Fort Hudson Health System, Inc. is seeking a Care Management professional to support our Care Management and Service Coordination program.
Responsibilities include completing assessments and providing pre-enrollment education for insurance programs, with duties split between field visits and in-office work. The ideal candidate holds an RN license and has experience in community health or a strong willingness to learn.
