Job description
South Park Health Center in Fairplay, CO is seeking a full-time Physician Assistant or Family Nurse Practitioner to provide outpatient primary care across the lifespan, from pediatrics to geriatrics.
You will practice independently with strong physician support, manage chronic conditions, same-day needs and preventive care in a rural community, delivering high-quality patient-centered care with a collaborative clinic team.
