A dynamic community-based health center in Manhattan is seeking a dedicated Nurse Practitioner to join its multidisciplinary team. This full-time role focuses on providing compassionate, comprehensive care to adult patients, including those living with HIV, in an outpatient setting. The center is a Federally Qualified Health Center (FQHC) and a recognized Patient-Centered Medical Home, offering a wide range of bilingual health services.
Key Responsibilities:
Deliver primary care services with a focus on HIV treatment and prevention in an ambulatory care environment.
Participate in the center s on-call rotation, providing telephone coverage as scheduled.
Support quality improvement initiatives and contribute to clinical program development.
Collaborate with other healthcare professionals to ensure continuity of care and optimal patient outcomes.
Engage in staff education and development activities to promote best practices.
Maintain accurate and timely documentation in the electronic health record system.
Requirements:
Active New York State license as a Nurse Practitioner.
Must hold an active NYS Medicaid/Medicare provider number.
Strong communication and interpersonal skills; bilingual Spanish is a plus.
Minimum of 2 3 years of clinical experience in adult medicine preferred.
Willingness to work toward HIV certification if not already certified.
