Job description
AbsoluteCare Medical Center & Pharmacy is seeking a physician to provide HIV and chronic infectious disease care in an outpatient setting as part of an integrated care team. You will develop diagnostic and treatment plans, educate patients, and coordinate with social work, pharmacy and other specialists to deliver whole-person care.
The role emphasizes evidence-based medicine within the PCMH model, patient education, and seamless transitions across care settings.
