Job description
AdventHealth in Palm Coast, FL is seeking a Discharge Planning Supervisor to oversee daily discharge planning and social services across inpatient, observation, and emergency departments. You will partner with Care Management and facility leaders to remove barriers and coordinate progression of care.
You will participate in rounds, support staff development, monitor documentation quality with dashboards, and help with complex transitions.
