Job description

White Oak Medical Center in Maryland seeks an experienced Inpatient Care Navigator/Discharge Planner (RN or SW) for the Care Navigation department. The role focuses on psycho-social assessment, discharge planning, and coordination with care teams to ensure timely patient discharge.

The RN requires nursing licensure and 1–3 years of clinical experience; SW requires BS/W or MSW with 3+ years in diverse clinical settings. BLS, Cerner EMR, and Allscripts experience preferred.
