Job description
The Case Management (CM) Registered Nurse (RN) position is based at the Audie L. Murphy Memorial Veterans' Hospital in San Antonio, TX. The CM RN delivers patient management, care coordination, and discharge planning for inpatients and outpatients, supporting Veterans, families, and caregivers with complex care needs.
