Job description
MedStar Health is currently seeking an Inpatient Case Manager to join our team. This role facilitates quality, cost-effective, patient-centered care from pre-admission through post-discharge.
As a member of the Case Management team, you will monitor care for selected patient populations during the acute hospital stay, promote resource utilization and coordinate plans of care with multidisciplinary teams to meet individualized outcomes.
