Job description
LiveWell Homecare Agency, LLC is seeking a licensed pharmacist to provide comprehensive inpatient pharmacy services, including Medication Therapy Services protocols that support clinical intervention and medication optimization.
In a collaborative hospital setting you’ll assess, monitor, initiate, and modify medication use based on patient condition and age-specific criteria, supervise supportive personnel, ensure accurate preparation and dispensing, and serve as a medication therapy resource
