Job description
Stony Brook University Hospital is seeking an Inpatient Diabetes Educator to provide CDCES diabetes education assessment and DSME to patients and families, collaborating with care teams to develop and evaluate patient-centered plans during hospitalization.
The role requires NYS RN licensure, CDCES certification, 2–5 years diabetes program experience, and involves duties from 1:1 DSMES in inpatient settings, discharge planning, and maintaining records in EMR.
