Job description
Kaiser Permanente seeks a Licensed Social Worker for our Honolulu inpatient team. In this role, you’ll provide psychosocial assessments, crisis intervention, and discharge planning for diverse patients in a fast-paced hospital environment. You will coordinate care with physicians, nurses, and community partners to ensure safe transitions and access to needed resources. Using evidence-based practices and electronic health records, you’ll develop treatment plans, lead family and patient meetings, and advocate for patient needs while supporting a culture of innovation, collaboration, and continuous improvement.
