Job description
Encompass Health seeks an RN Case Manager to coordinate patient-centered rehabilitation care in our inpatient rehab hospital. This role involves managing clinical cases from admission through discharge, developing and updating individualized care plans, and collaborating with an interdisciplinary team.
You will educate patients and families, support safe transitions of care, and advocate for optimal outcomes.
