Job description
Encompass Health seeks a Clinical Pharmacist to support our inpatient rehabilitation hospital within the Healthcare & Medical Services department. In this role, you will provide evidence-based pharmacotherapy, medication therapy management, and medication safety oversight for medically complex rehab patients. You will collaborate closely with physicians, nurses, and the interdisciplinary team to optimize outcomes, lead medication reconciliation, counsel patients and families, and support quality and stewardship initiatives.
