Job description
Encompass Health seeks a Clinical Pharmacist to deliver evidence‑based pharmacotherapy in our inpatient rehabilitation hospital. The Clinical Pharmacist will verify and dispense medication orders, optimize drug therapy, and monitor for interactions and adverse effects. In close collaboration with physicians, nurses, and the rehab team, you will develop patient‑centered medication plans, perform medication reconciliation, and educate patients and families on safe medication use. This role supports medication safety initiatives, quality improvement, and a nurturing, high‑quality patient care environment.
