Job description
Encompass Health seeks a Clinical Pharmacist to support our inpatient rehabilitation hospital within the Healthcare & Medical Services department. In this on-site role, you will deliver comprehensive clinical pharmacy services, including medication review, reconciliation, and therapeutic monitoring for complex rehab patients. You will collaborate closely with physicians, nurses, and therapists to optimize pharmacotherapy, prevent adverse events, and support safe transitions of care. Drawing on Encompass Health’s culture of high-quality, patient-centered care, you will educate patients and staff, contribute to clinical rounds, and participate in quality and medication safety initiatives.
