Job description
UAB Medicine is seeking a Registered Nurse Care Coordinator (RNCC) to join a Magnet-designated nursing team. You will coordinate services, assess post-discharge needs, and facilitate referrals for in-home care while supporting discharge planning and care coordination across inpatient and ambulatory settings.
The role requires a nursing degree and current RN license, with BLS certification and department-specific ACLS/PALS requirements.
