Job description
Aurora St. Luke’s Medical Center in Milwaukee is seeking an RN Case Manager to coordinate inpatient discharge planning and ongoing case management. You will collaborate with patients, families, and the care team to ensure safe transitions through the hospital stay and align with regulatory standards.
The role requires an active RN license and at least 2 years of clinical nursing experience; a Bachelor’s Degree in Nursing is preferred. Located in Milwaukee, WI, this semi-remote?
