Job description
MedStar Montgomery Medical Center in Maryland is seeking an Inpatient Case Manager to join our social work team. The Social Worker serves as a patient liaison between hospital and community, facilitating a smooth transition from acute care to post-hospital services.
You will conduct biopsychosocial assessments, develop discharge plans, counsel patients and families, and collaborate with health care teams to ensure timely and effective patient flow.
