University of Colorado Anschutz Department: Hemophilia& Thrombosis Center Job Title: Instructor – NursePractitioner/Physician Assistant Position #: –Requisition #:41197

Job Summary:

The Hemophilia and Thrombosis Center is an interdisciplinaryclinical and research program for children adults with bleeding orclotting disorders. The person in this position has primaryresponsibility to implement the clinical care, case management andprogram coordination for patients with bleeding and clottingdisorders. The position is 1.0 FTE.

Key Responsibilities:

Provide comprehensive clinical care to hemophilia,thrombosis, and stroke patients, including patient assessment,diagnosis, treatment planning, and follow-up.

Conduct patient evaluations, including medical histories,physical examinations, clinical trial assessments, and review ofdiagnostic laboratory tests and imaging.

Develop and manage individualized patient care plans basedon clinical findings, medical history, and established clinicalcare guidelines.

Collaborate with multidisciplinary teams includingphysicians, pharmacists, physical therapists, social workers, andother specialists to coordinate and optimize patient care.

Provide patient and family education and support,including self-care instructions, phone triage, and educationrelated to hemophilia, thrombosis, and thrombophilia.

Maintain accurate clinical documentation and patient data,including proficiency with EPIC and supporting data qualityassurance and clinical program requirements.

Participate in HTC outreach and professional activities,including regional and national meetings, remote clinics, camps,education days, community events, and in-service training forhealthcare staff and students.

Qualifications:

Minimum Qualifications:

Applicants must meet minimum qualifications at the time ofhire.

Current Certification as Nurse Practitioner or Physician AssistantColorado License with prescriptive writing capability

Preferred Qualifications:

Two (2) or more years of general clinical experience.

Two (2) or more years of hematology clinical experience.

Experience with clinical research.

Familiar with point of care ultrasound

Knowledge, Skills and Abilities:

Skills in working in a collaborative, multidisciplinaryenvironment with a medical, psychosocial and administrative team tofacilitate and develop patient treatment plans.

Experienced caring for both pediatric and adultpopulations.

Working knowledge of caring for patients with chronicconditions.

Professional demeanor and experience that demonstrates problemsolving skills and the ability to manage multiple tasks and workindependently with minimal supervision.

Excellent organizational, written and verbal communication skillswith the ability to take direction.

Applicants will possess the qualities of being detail-oriented,self-motivated and a fast learner with good time managementskills.

Proficient in electronic medical records systems.

Conditions of Employment:

Anticipated Pay Range:

The starting salary range (or hiring range) for thisposition has been established as HIRING RANGE:

$97,000-112,000.

The above salary range (or hiring range) represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting. This position is not eligible for overtime compensation unless it is non-exempt.
