This role includes insurance verification, authorization, data entry, and coordination of care, while serving as a key point of contact for patients, families, and referral sources.Key ResponsibilitiesOversee daily intake and referral operations to ensure timely patient admissionsVerify insurance eligibility, authorizations, and reimbursement accuracyCoordinate patient transitions to home health servicesProvide direct oversight of patient education and preparation for home health, plan of care initiation, and coordination of care with multiple service providers.Maintain strong relationships with referral sources, case managers, and healthcare partnersTrack and report referral trends and intake metricsSupport pricing discussions and payer communication as neededQualificationsHealthcare experience required, preferably in home health or hospice intake/referralsStrong knowledge of insurance verification, authorizations, and reimbursement processesExcellent communication, customer service, and relationship-building skillsAbility to work independently with strong organization and multitasking skillsNursing background preferred, but not required
