We are looking to hire an Intake Coordinator to join our Morrow, Georgia home-health office team. We believe in Bridgeway Home Health's mission and strive to do the right things, the right way, all the time.What We Offer:MedicalPrescription Drug PlanTelehealthDentalVisionVoluntary Short-Term DisabilityVoluntary Long-Term DisabilityVoluntary Life Insurance401kPaid Time OffMileage ReimbursementEmployee Referral ProgramOverview:The Intake Coordinator is responsible for accepting incoming calls for new referrals for clinical services provided to home care patients within an assigned geographical area.Receives clinical information on new patient referrals to the home health agency.Collects all data pertinent to the request for admission to include demographic data, medical history, medication, physical, discharge summary, labs, physician orders and insurance informationVerifies insurance/payer's source via DDE or specific systemEnsures referral acceptance and communication within 15 minutes of receiving referralNotifies clinical designee responsible for scheduling and authorizationsContact patient/clinical designee prior to admission to ensure service locationLiaisons between field, medical, office and referral source staffObtains authorization from insurance companies and ensures physician licensure complianceConducts follow up on current active patient hospitalizations and communicates with clinical supervisors to obtain orders as neededMaintains non-admit logAttends in-services and meetingsRequirements:High school diploma or GED equivalentPrior clinical home health experience preferred; physician practice, hospice experience a plusKnowledge of Medicare, TennCare, state and federal regulations, and accreditation standards as they relate to clinical home healthProficient in Microsoft Office Suite including Outlook, Word and ExcelFamiliarity with medical terminologyExcellent verbal, written, organizational, collaboration, multi-tasking and time-management skillsStrong initiative, follow through, process orientation, attention-to-detailDemonstrated confidentiality, professional and business acumen
