You will serve as the first point of contact for patients and referral partners, ensuring timely admissions, verifying insurance, coordinating care services, and delivering a seamless transition into home health. You will:Oversee all daily patient intake and referral operationsManage the full intake process, including insurance verification and authorizationCoordinate and manage patient scheduling to ensure timely admissions, evaluations, and home health visitsOptimize clinician schedules while balancing patient needs, geographic considerations, and staffing resourcesServe as a key point of contact for patients, families, physicians, and referral sourcesReceive, process, and track incoming referrals to support efficient patient onboardingCommunicate with clinical and administrative teams to facilitate smooth transitions into careMonitor scheduling changes and proactively resolve conflicts, delays, or coverage concernsTrack and report referral data, trends, and performance metricsIdentify opportunities to improve services and patient access to careOur Commitment to YouPerform meaningful work focused on improving patient care and outcomesWork with a collaborative and mission-driven team that celebrates your successOngoing professional development and trainingOur RequirementsHigh school diploma or equivalent requiredMinimum of one (1) year of experience in healthcare scheduling, intake, medical office administration preferredHome Health experience a plusExcellent communication, customer service, and relationship-building skillsKnowledge of healthcare regulations and business operationsCurrent CPR certification
