Intake Coordinator needed for a contract opportunity with Yoh’s client located in San Francisco, CA! **WORK FROM HOME**
Top Skills Should You Possess:
Minimum one (1) year of experience in Triage, Intake or Care Coordination
What You’ll Be Doing: Ensure complex populations get connected to the most appropriate services available
Provide the primary intake and triage services for telephonic referrals and inquiries to all care coordination programs within the organization and the external provider network
Manage incoming calls to the Care Management Intake Line
Receive and process inquiries for all Care Management programs from internal referrals, community providers, emergency departments, primary care providers and other agencies
Provide outstanding customer services to internal and external customers
Triage and process referrals into the appropriate CM programs, actively seek consultation from clinical supervisors when needed to escalate a referral and support the Notice of Action (NOA) process
Effectively manage the Community Supports services referral process including referrals from SF Medical Respite
Triage and de-escalate calls from members who may be in crisis
Schedule: Monday – Friday, 8am – 5pm or 8:30am – 5:30pm (Pacific)
What You Need to Bring to the Table: Associates or Bachelors Degree in related field; or 4 years of relevant experience
Minimum one (1) year of experience in Triage, Intake or Care Coordination
Bi-cultural, bi-lingual language skills preferred
Experience working with people with complex health needs
Working knowledge and use of Trauma Informed language and principles a plus
Working knowledge and use of Harm Reduction language and principles a plus
Working knowledge and use of Person-Centered language and principles a plus
Experience with de-escalation techniques
Telephonic Case Management experience a plus
Excellent verbal and written communication skills
Excellent computer skills
