Job description
Oak Street Health is seeking a Behavioral Health Specialist to provide high-quality, evidence-based care to adults on Medicare in an integrated primary care setting. In this role, you will conduct comprehensive behavioral health assessments, deliver short- and long-term therapy, and collaborate closely with primary care providers, nurses, and care teams to develop coordinated treatment plans. You will manage a diverse caseload, support patients with chronic and complex conditions, respond to crises, and contribute to quality improvement initiatives that advance patient-centered, preventative care in underserved communities.
