Job description
Grace Health is seeking an Integrated Behavioral Health Consultant to deliver brief, evidence-informed interventions within our PCBH model in a primary care setting. You will work as an integrated member of the health care team to improve both physical and mental well-being for patients across ages and families.
You will collaborate with medical staff, provide real-time feedback to PCPs, and document all encounters in the EHR.
