Job description
The Primary Care Behavioral Health (PCBH) model is integrated into Spira Care's primary care practice in the Kansas City area. The Behavioral Health Consultant (BHC) collaborates with medical providers to address both physical and mental health needs and to promote patient self-management.
The BHC will deliver brief, evidence-based interventions, participate in team meetings, and educate patients about diagnoses and healthy lifestyle changes while ensuring quality documentation and coordination
