Job description
CareSource/Commonwealth Care Alliance (CCA) seeks an Integrated Care Clinical Manager (MA) to monitor and manage care for dually-eligible enrollees with complex medical and social needs. The role focuses on coordinating services and improving health outcomes through home visits and community-based support.
You will lead an interdisciplinary team, collaborate with clinicians, and ensure effective care transitions while addressing beneficiaries’ cultural and linguistic needs.
