Job description
PeaceHealth in Eugene, Oregon, seeks a Family Nurse Practitioner (0.8–1.0 FTE) to join the Unified Primary Care Behavioral Health team. The role combines primary care with behavioral health within an integrated care model, serving patients with complex medical and behavioral health needs.
You will collaborate with a multidisciplinary team, including psychiatrists, addiction medicine specialists, and care coordinators, to provide holistic care and coordinate with community resources.
