Job description
Oak Street Health seeks a Behavioral Health Specialist to provide evidence-based counseling and support to Medicare adults in underserved communities. In this role, you will complete comprehensive behavioral health assessments, develop individualized treatment and safety plans, and deliver brief therapy interventions. You’ll collaborate closely with primary care providers, nurses, and care teams to integrate behavioral health into primary care, monitor progress, and connect patients to community resources, helping improve access, outcomes, and patient well-being in a supportive, patient-centered culture.
