nBase Schedule: 35-hour work week. Monday – Friday 8:30-4:30. With occasional nights and weekends including rotation of on call.
nJOB DESCRIPTION: The Intensive Case Planner provides comprehensive, strength-based case management services to families involved with the child welfare system. This role is focused on ensuring child safety, family stabilization, and long-term well-being through collaborative planning, advocacy, and therapeutic support. The Intensive Case Planner works closely with families, the Department of Family and Community Services (DFCS), legal stakeholders, schools, mental health providers, and other community-based organizations.
nSPECIFIC RESPONSIBILITIES INCLUDE:
nnnMaintain a caseload of families with elevated service needs and/or high-risk indicators.
n
nnConduct regular home and community-based visits (minimum twice per month or as mandated).
n
nnEngage children and families using a trauma-informed and culturally responsive approach.
n
nnComplete comprehensive family assessments and individualized service plans in accordance with DFCS.
n
nnCoordinate services including mental health, substance use, domestic violence, education, housing, and employment supports.
n
nnFacilitate family team conferences and case planning meetings with internal and external stakeholders.
n
nnMaintain timely and accurate documentation in electronic case records (e.g., Connections).
n
nnSubmit all required reports, progress notes, and court reports in accordance with DFCS and agency guidelines.
n
nnEnsure all contacts, consents, and assessments meet regulatory and agency standards.
n
nnCollaborate with DFCS, legal representatives, and service providers to ensure coordinated care.
n
nnCoordinate with clients and treatment providers to secure placement for children requiring secured detention.
n
nnAttend and provide testimony at Family Court proceedings when required.
n
nnAdvocate for services that strengthen family functioning and promote child well-being.
n
nnRespond promptly to emergencies, including safety concerns, hospitalizations, or child removals.
n
nnWork collaboratively with supervisors to assess risk and implement safety interventions.
n
nnParticipate in individual and group supervision.
n
nnAttend case conferences, training sessions, and team meetings.
n
nnMaintain certification or proficiency in evidence-based models and/or mandated programs (Mandated Reporting).
n
nnMaintains self in a professional manner at all times and adheres to the policies and procedures of the Agency.
n
nnPerforms various related tasks as assigned or required.
n
nnPunctual and regular attendance is an essential responsibility of each employee. Employees are expected to report to work as scheduled, on time and prepared to start working.
n
nnEmployees are expected to be available to work occasional evening and weekend events as assigned, based on the needs of the program.
n
nnEmployee must be available to participate in a rotating on-call schedules as required by the program. This includes responding to urgent matters outside of regular business hours, including evenings, weekends, and holidays.

n nREQUIRED QUALIFICATIONS:
nnnA Bachelor’s Degree in Social Work, Nursing, Psychology or a closely related Human Service field
n
nnAND
n
nnTwo (2) years relevant experience working with children/adults with mental health/ behavioral health concerns
n
n nAND
nnA valid New York State Driver’s License and reliable transportation required to conduct home/community based visits
n nAND
nnnKnowledge of DFCS Preventive Services model and evidence-based interventions.
n
nnFamiliarity with state and local child welfare regulations and systems.
n
nnProven strong organizational, assessment and communication skills.
n
nnExperience in securing cooperation of, and working effectively with individuals and agencies in the community.
n
