Tidepoint Healthcare Staffing is hiring a Travel RN - Director of Care Coordination
Shift: Non-exempt, Days, 7a-3p, 8x5, 40hrs per week, Mon-Fri
Duration: 13 weeks
Requirements:
Active California RN License
Minimum 5 years of experience in an acute healthcare setting (required)
Minimum 5 years of progressively responsible leadership/administrative experience in Case Management, Care Coordination, Utilization Management, or a closely related function (required)
Previous leadership experience in Care Coordination (must have/firm)
Demonstrated experience leading interdisciplinary clinical/operational programs, staff, performance improvement, and regulatory or accreditation readiness (required)
Case Management certification such as CCM or ACM (preferred)
BSN (required)
Master’s degree in Nursing, Health Administration, or Business Administration (preferred)
Other Details:
Advanced knowledge of case management, care coordination, utilization management, discharge planning, patient progression, and transitions of care in an acute-care environment
Acute care hospital setting; bed size 150-500
EMR used: EPIC
1 position available
Up to 36 hours of call off available to the facility at no cost during the assignment

The Pay Range for this position is $4,700.00 - $5,100.00. Please note that all pay rates are gross weekly pay estimates. Actual compensation is influenced by a wide array of factors including, but not limited to, shift, skill set, location and stipends, etc.
