Locums - Geriatric Medicine Physician Kalispell, Montana (IMLC Accepted) Quick HighlightsLocation: Kalispell, MTAssignment Type: Ongoing Locum TenensStart Date: ASAPSchedule: 3 days per week (Preferred: Tuesday-Thursday)Hours: 7:00 AM - 5:00 PM (10-hour shifts)No Night Call - Covered by HospitalistsDedicated APP Support EMR: MediTech AvailabilityASAP startOngoing coveragePreferred schedule: Tuesday, Wednesday, Thursday10-hour daytime shifts (7:00 AM - 5:00 PM)No night call responsibilitiesBeeper call listed; hospitalists cover overnight call30-minute response time if call coverage is assigned Required LicensesActive Montana Medical License OR Active IMLC Required Certifications & QualificationsBoard Certified or Board Eligible in Internal Medicine or Family Medicine BLS CertificationACLS CertificationExperience in Skilled Nursing Facility (SNF), Long-Term Care (LTC), Transitional Care, or Post-Acute MedicineExperience caring for geriatric and medically complex patients strongly preferred Practice DetailsCoverage for Skilled Nursing Facility (SNF) and Long-Term Care (LTC) patientsManage both short-term rehabilitation and long-term care residentsAdmit new patients and complete History & PhysicalsConduct CMS-required physician visits and recertificationsManage chronic conditions, preventive care, dementia, frailty, and complex geriatric illnessesEvaluate acute changes in condition to reduce ED transfers and hospital readmissionsMedication management and review of pharmacist recommendationsDevelop and update interdisciplinary care plansParticipate in care conferences, family meetings, and goals-of-care discussionsCoordinate transitions between hospital, SNF, LTC, specialty care, and community providersComplete all regulatory documentation and compliance requirements Practice Information99-bed unit12-14 patients per shiftAdmissions requiredAssigned rounding patientsTrauma Level IIIDedicated Advanced Practice Provider (APP) support Ideal CandidateA Board-Certified or Board-Eligible Internal Medicine or Family Medicine physician with experience in Skilled Nursing, Long-Term Care, Transitional Care, or Post-Acute Medicine who enjoys caring for medically complex geriatric patients.
