Facility:
This facility operates as a PACE program (Program of All Inclusive Care for the Elderly), serving exclusively adults over the age of 55. The program combines in-clinic and community-based care, supported by an 11-person interdisciplinary team dedicated to providing comprehensive, coordinated care for the elderly population.
Opportunity:
Board Certified - Internal Medicine - Geriatric Medicine
Job Details:
Outpatient, clinic-only position with Monday through Friday availability; 3-4 days per week will also be considered
Shift times are 8:00 AM to 5:00 PM with 8 guaranteed hours per day
Minimum of 2 weeks per month required; full-time availability preferred
Provider must have geriatric experience as the majority of the patient population are elderly adults over the age of 55
4-6 patient encounters per shift
Combination of in-clinic visits and community-based visits to patient residences, including assisted living facilities, skilled nursing facilities, and personal residences
Off-site community visits will include a nurse from the team accompanying the provider
Most patient management is handled through the EMR, with other team members coordinating visits and care in the community
Work alongside an 11-person interdisciplinary team in a collaborative care environment
Active Oregon medical license required; no IMLC accepted
Board certification or true board eligibility required
Active DEA required
BLS certification required
November start date with ongoing need
Compensation:
Competitive hourly rates with weekly pay and malpractice included.
