Post-Acute Care NP/PA Opening Auburn, NYSyracuse 30mFull Time Nurse Practitioner or Physician AssistantPosition Highlights:- Full-time Post-Acute Care opportunity- 5-day-per-week schedule- Round in skilled nursing facilities- Provide comprehensive care to a diverse post-acute patient population- Perform medical histories, physical exams, and comprehensive patient assessments- Develop and adjust individualized treatment plans- Monitor patient progress and coordinate care to optimize outcomes- Collaborate with physicians, nursing staff, and multidisciplinary care teams- Educate patients and families on health management and preventive care- Strong clinical support and training- Autonomy with uncapped productivity earning potential- New graduates welcome to applyQualifications:- Nurse Practitioner or Physician Assistant- Active New York NP or PA license- Active DEA registration- Post-acute, acute care, Emergency Medicine, or outpatient clinical experience preferred but not required- Strong clinical and communication skills- Collaborative, team-oriented approachCompensation & Benefits:- Estimated annual compensation of $130,760-$154,911- Fee-for-service compensation model- Uncapped productivity income potential- Medical, dental, and vision insurance- 401(k) retirement plan- Health savings account- Flexible spending account- Flexible time off- Professional liability insurance
