Post-Acute Care NP/PA Opening Bronx, NYFull Time Nurse Practitioner or Physician AssistantPosition Highlights:- Full-time post-acute care opportunity- 5-day-per-week schedule- Round in skilled nursing facilities throughout the Bronx- Autonomy with strong clinical and operational support- Dedicated night call coverage for improved work-life balance- Collaborative multidisciplinary team environment- Care for a diverse post-acute patient population- Strong earning potential with uncapped productivity income- Professional growth and leadership opportunitiesResponsibilities:- Perform comprehensive patient assessments, including medical histories and physical examinations- Develop, implement, and adjust individualized care plans- Monitor patient progress and coordinate care to optimize outcomes- Collaborate with physicians, nursing staff, and other healthcare professionals- Educate patients and families on health management and preventive strategies- Maintain accurate and timely medical documentationQualifications:- Graduate of an accredited Nurse Practitioner or Physician Assistant program- Active or eligible New York NP or PA license- Active DEA registration- Experience in post-acute care, acute care, emergency medicine, or outpatient practice preferred but not required- Strong clinical foundation and knowledge of healthcare regulatory standards- Excellent communication and interpersonal skills- Collaborative, team-oriented approachCompensation & Benefits:- Estimated compensation of $132,000-$156,000 annually- Fee-for-service compensation with uncapped productivity potential- 401(k) retirement plan- Medical, dental, and vision insurance- Flexible Spending Account (FSA)- Health Savings Account (HSA)- Flex Time Off- Professional liability insurance
