This outpatient primary care practice provides comprehensive, relationship-based care to a predominantly adult and senior patient population , supported by a collaborative, physician-led care model. Advanced Practice Providers play a key role in delivering preventive care, managing chronic conditions, and building long-term patient relationships.
The practice structure emphasizes support, autonomy, and sustainability , allowing APPs to focus on high-quality care without inpatient or hospital-based responsibilities.
