APP - Internal Medicine Location: Stayton, OR Position Highlights•Join a committed and collaborative healthcare team in a community-based, not-for-profit healthcare system. •Opportunity available for an Advanced Practice Provider (NP or PA) to join an Internal Medicine primary care clinic. •Full-time, 40 hours per week, salary-based position. •Internal Medicine clinic focused on adult primary care, including management of complex medical conditions and chronic diseases. •Serve a diverse adult patient population with a broad range of healthcare needs. •Work alongside physicians, medical assistants, behavioral health clinicians, care coordinators, and community health workers. •Supportive and collegial environment focused on compassion, patient education, collaboration, and patient-centered care. •Participate in quality improvement initiatives, virtual care integration, and patient education programs. •Community-focused healthcare system committed to providing high-quality care close to home. Healthcare System•Community-based, not-for-profit 40-bed acute-care hospital with multiple outpatient clinics. Clinics include: oFamily Medicine oInternal Medicine oCardiology oGeneral Surgery oPulmonology & Sleep Medicine oOrthopedics oWomen's Health oPodiatry •Multiple Rural Health-designated primary care clinics serving surrounding communities. •Located in Stayton, Oregon, in the foothills of the Cascades. •Convenient access to Oregon's mountains, coastal beaches, wineries, and the Willamette Valley. Responsibilities•Provide comprehensive primary care to an adult patient population. •Diagnose and manage acute and chronic medical conditions. •Provide preventive care and health education. •Develop individualized treatment plans and coordinate patient care. •Collaborate closely with physicians and multidisciplinary healthcare professionals. •Participate in clinic-wide quality improvement and patient care initiatives. •Utilize virtual care tools and other innovations to enhance patient access and outcomes. Qualifications•Active Oregon NP or PA license. •Current DEA license. •Current BLS or ACLS certification.
