Job description
Evolve Therapy Services in Key West, FL is seeking a licensed Physical Therapist to join our in-house Skilled Nursing Facility program. You will provide professional PT services to residents in accordance with standards of practice, company procedures, and CMS expectations.
The role includes evaluating residents, developing treatment plans with short and long-term goals, documenting findings in NetHealth, and collaborating with families and the interdisciplinary team.
