Role Overview
We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across our partner communities. This role offers steady, predictable patient volume and the opportunity to deliver high-quality care directly where seniors live.
What You’ll Do
Routine & urgent assessments; chronic disease management
Post-discharge follow-ups, med reconciliation, labs/diagnostics
Coordinate with facility staff, specialists, families/POAs
Same-day documentation; CMS/regulatory compliance
Schedule & Setting
Full-time position with a structured weekday schedule. You'll deliver on-site care across the LA and OC area with strategically clustered routes designed to minimize travel and maximize time with patients, combined with telehealth visits for a flexible, hybrid schedule.
Requirements
Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
Geriatrics or facility-based experience preferred; EMR proficiency
Fluent in Korean and English (required)
Benefits
Compensation
Annual compensation range: $140,000–$160,000 (based on experience, schedule, and patient volume)
Additional earning opportunities available based on coverage and panel needs
W-2 with full benefits available; mileage reimbursed per policy
