Job description
Kaiser Permanente seeks an LCSW specializing in Crisis & Triage Management to support members through technology-enabled behavioral health services. In this hybrid role, you will conduct rapid risk assessments, provide brief crisis intervention, and develop safety plans via phone and digital platforms. You will coordinate higher levels of care, connect patients to community resources, and document all encounters in EHR systems. Partnering with clinicians and IT teams, you’ll help refine virtual workflows and leverage data to improve access, quality, and patient outcomes in a collaborative, innovation-driven environment.
