Job description
Collage Home and Community Rehabilitation Partners is seeking an experienced Licensed Clinical Social Worker (LCSW) for PRN work in Fullerton, CA. In this role you will assess, counsel, and develop individualized plans for medically complex patients receiving care in their homes and communities.
You will join an interdisciplinary team delivering patient-centered rehabilitation, with emphasis on durable outcomes and compassionate care in clients' own environments.
