Job description
St. Elizabeth Physicians in Hebron Conner Primary Care seeks a Licensed Clinical Social Worker (LCSW) to provide care management and biopsychosocial assessments within a primary care setting.
You will collaborate with the patient’s PCP to determine diagnosis and treatment, and provide brief therapy with follow-up. The role focuses on adult and geriatric patients, adherence to HIPAA, and close teamwork with the care team to improve outcomes.
